Cardiovascular diseases (CVD) are a major worldwide health problem, responsible for a third of all deaths. They generally refer to conditions in which blood vessels are narrowed or blocked, which can lead to heart attacks, chest pain (angina pectoris), or strokes. Cardiac biomarker assays have proven particularly useful in providing rapid diagnosis and risk assessment in people with acute coronary syndrome (ACS). Heart markers are proteins, hormones, enzymes, or other substances that are expressed by myocardial cells and released into the circulating blood in cell necrosis. Several routinely used biomarkers include creatine kinase (CK), creatine kinase-MB (CK-MB), myoglobin, cardiac troponin T (cTnT), and cardiac troponin I (cTnI). So far, troponins have proven to be the most sensitive and specific indicators of heart injury. Other useful biomarkers are NT-proBNP, which can be used to predict an increased risk of recurring events after a cardiac arrhythmia. In general, the use of more than one biomarker increases the early predictive value compared to the use of a single marker, and advances in technology will increase the use of multi-marker profiles to improve and possibly individualize the treatment of CVD in the future.
